Festoons

This 59-year-old patient traveled all the way from the UK to seek expert treatment for severe festoons. Festoons, or malar mounds, can create a persistent swollen appearance under the eyes, often resistant to conventional treatments. This patient came to see Dr. Kass complaining that when at work she felt that everyone was staring at the bags under her eyes. This 61 year old doctor’s wife gave a history of bilateral cheek swelling years after receiving hyaluronic acid filler to her cheeks and tear troughs. Besides her festoons which she wanted corrected, this lady had an obvious problem and was embarrassed by her severe tear trough deformities. At the time of her surgery, fat from the lower eyelids was transposed over the orbital rims and tear troughs to soften her appearance.
